Does Arizona Medical Clinic accept Medicare assignment?
Yes, the Clinic accepts assignment. This means the provider has agreed not to charge over the insurance company's allowed amount. However, often times there is co-insurance, or a co-payment which is not covered under the assignment agreement.
Will Arizona Medical Clinic submit my charges to my insurance company for me?
Yes, we will assist you by filing your claim with your Medicare or Commercial insurance plans. If you have a secondary insurance we will file this for you as well.
Who do I call if I have a question about my bill?
Questions regarding your bill should be directed to Business Services at (623) 876-3900 between the hours of 8:00am and 4:00pm, Monday through Friday, except Holidays.
How do I know if Arizona Medical Clinic accepts my insurance?
Contact your insurance company prior to scheduling an appointment to confirm provider participation with your plan.
What should I bring to my first appointment?
- Your insurance cards.
- The name and phone number of an emergency contact person.
- Any patient information forms that may have been sent to you for completion.
- All of your medications in their bottles, including nutritional supplements.
- Any old records or x-rays you may have in your possession. If you don't have your records, your physician will request pertinent records from your prior physician.
How do I request a refill for my medications?
For established medications call your pharmacy and request a refill. The pharmacy will obtain approval from your physician. Please allow 24 hours for your request to be completed. For new medications you must contact your physician's office.
What if I have an urgent medical concern and my physician's office is closed?
Go directly to a hospital emergency room for conditions that you believe are critical, or threatening to life or limb, or which may require extensive care. Arizona Medical Clinic offers a Same Day Care Clinic which is open after hours and on weekends. For other urgent concerns call our answering service at (602) 787-3265 and a health care provider will assist you. |
